WHAT IS THE PEYRONIE

Peyronie's disease or fibroplastic induration of the penis (in Latin "induracio penis plastica" – IPP) – is the disease during which the male's sexual organ, the penis, gets curved due to progressive fibrous changes in the protein membrane of the penis.
This disease is named after a French surgeon Francois Peyronie, who described it in 1743. Most often, men safer from it between the ages of 30-60, 65.

Peyronie's disease – is a pathology that is typical only for the males. The disease is characterized by the appearance of painful erection due to the fibrous changes in the protein membrane.
The illness is rare and it usually occurs in men ages 40-60. The main reason for the appearance is the occurrence of plaques in the membrane as well as benign neoplasms on the penis or urethra.
The illness tends to progress and it can lead to complete incapability of having a sexual act.
In some cases, this illness can have a negative impact on conception but it's present only in those cases where the penis is curved so much that it cannot enter the female sexual organ.
This is why it's necessary to see a specialist after noticing the first symptoms (the presence of a small seal that is easy to detect by touch, painful sexual intercourse, and deformation during each erection). The treatment is surgical.

THE REASON FOR APPEARANCE OF PEYRONIE

The reason for the appearance of Peyronie is unknown. Most likely, it is due to the repeated microtrauma of the protein membrane of the cavernous bodies. The disease starts with an acute inflammatory process in the result of which dramatically increases the proliferation of fibroblasts of the membrane, some of which differentiate into myofibroblasts, increases the deposition of collagen and fibrin. Ongoing inflammation leads to remodeling the connecting tissue into the fibrous plaque which may lead to the curvature of the cavernous bodies.
Risk factors for Peyronie's disease are diabetes, arterial hypertension, lipid metabolism disorders, coronary heart disease, erectile dysfunction, smoking, and alcohol abuse.

THE PROCESS OF PEYRONIE'S DISEASE

There are two stages of this disease: the stage of acute inflammation and the fibrotic stage.
The stage of acute inflammation can reveal itself with the pain in the penis at rest, painful erection, as well as the appearance of "soft" plaques and curvature of the penis.
In the fibrotic stage, a dense palpable plaque is formed, which can subsequently calcify, which indicates the stabilization of the disease.
With time, the worsening of the condition was detected in 30-50% of patients and stabilizing in 47-67%. Spontaneous improvement in the patient's state happens only in 3-13% of cases. Improvement is possible only in the early stages of illness. After formation, especially if the fibrotic plaque has been calcified, spontaneous improvement happens very rarely.

DIAGNOSES AND TREATMENT OF PEYRONIE

A first diagnosis is made based on the results of the external examination and palpation of the penis. Diagnoses confirmation can be made after the x-ray or ultrasound of the penis which determines the size and structure of the fibrous plaques.
Clinically, the illness manifests itself by the curvature of the penis and pain during the erection up to the impossibility of sexual intercourse. About 30% of patients with Peyronie's suffer from erectile dysfunction.

Medical treatment of Peyronie's disease:
Orally: Vitamin E, Tamoxifen, Potassium Aminobenzoate, Colchicine, etc.
Intracavernous injections Lydasum, Interferon, Hydrocortisone, Collagenase, Lidocaine, etc.
Physiotherapy: electrophoresis with Lydasum or Chymotrypsin, LNP-therapy (Local Negative Pressure).
Also, in modern medicine, recently started using shock wave therapy to treat Peyronie's. This method successfully substitutes surgery. The therapy is carried out by non-invasive contact using a direct impact on the treated area.

If the surgical method is chosen, radical excision of the fibrous plaque is made. Sometimes it's necessary to have plastic surgery on the place of defect which forms on the cavernous body.

POSSIBLE REASONS FOR THE APPEARANCE OF PEYRONIE

There is a wide specter of traumas of the penis. For example, during sexual intercourse, getting hit or falling on the hard surface, micro-traumas that are unseen to the eye and not accompanied by pain.

  • Micro-traumas can be caused during sexual intercourse;
  • Closed fracture of the penis, during which, the skin is not broken but the hemorrhage occurs;
  • The presence of narrowing of the lumen of blood vessels, which usually happens with arteriosclerosis disease;
  • Wide specter of connective tissue illnesses;
  • Gout – an increase in the content of uric acid in the blood;
  • Deficiency of Vitamin E and calcium in the body;
  • The use of medications for treating the impotence; - Increased level of serotonin in the body;
  • Increased level of serotonin in the body;
  • The age of a male. In the aging process, the tissues of the body lose their elasticity and are most of all are subjected to traumas.

PENIS CURVATION AS A BIRTH DEFECT

This disease can be congenital very rarely, for example, with pathologies of the structure of the protein-membrane or abnormally short urethra.
The main difference between congenital and acquired disease is the absence of plaques, and deformation is developed due to the tension in the cavernous bodies.
Minor deformations are not a serious problem and don't need surgery. In the world, there are about 3% of males that suffer from congenital Peyronie's disease.

TYPES OF PEYRONIE

TYPES OF PENIS DEFORMATIONS:

Dorsal – the penis is curved up;
Ventral – penis is curved down;
Lateral – the penis is curved to the side.

HOW TO KNOW IF YOU HAVE PEYRONIE'S

Practically all patients complain about the following symptoms of the illness:

  • Painful erection and problems with it;
  • Significant curvature of the penis;
  • Existence of plaques which can be easily felt with fingers;
  • Problems with having sexual intercourse, pain.

It should be noted that in some cases the penis is so curved that a simple sexual act becomes practically impossible and causes serious discomfort to a partner.

In this case, newly formed plaques with the consistency of cartilage can interfere. They may get up to 3 cm in size.

Usually, large plaques are in the protein-membrane closer to the body of the coronal groove of the penis, close to the pubic area. Depending on where the plaques are, the urologist concludes about the type of penis curvature.

The illness has two stages:

  • Acute (unstable). At the beginning of the illness, there is no curvature of the penis or it's very insignificant, but the erection is painful. Under the skin of the penis, the scar plaques can be felt with fingers. They can increase or decrease in size. Usually, the acute stage lasts for 6-12 months.
  • Chronic (stable). The pain is almost gone but the curvature of the penis increases. The penis might get shorter. Sometimes it doesn't get in the way of the sexual act and sometimes it makes it totally impossible. Some men develop problems with erection. Almost half of the men (42% of cases) have a dorsal penis curvature (to the top), rarer (37% of cases) have it curve to the left or right.

When should you see a doctor? See the urologist-andrologist if you notice that your penis is curved and it gets in the way of having a sexual act or causes pain and erection dysfunction. You must take a photo of your penis when it's in the stage of erection, from the top, to the side, and in front and bring the photos to a doctor. A doctor will take a look at the genitals and if needed, will inject the substance into the penis that will cause the erection to see it in the state of erection and determine the level of curvature. For scar-tissue detection, an ultrasound or MRI is prescribed.

Sometimes, Peyronie's disease can have an easy form. Suddenly, the patient might find plaques on his penis but they go away on their own six months or one year after without any type of treatment.
Plus, the symptoms of this disease may continuously progress to better or worse during one and a half years after its first appearance. Unfortunately, one-year passes and medicated treatment might not give the desired effect so the doctors use the surgical procedures.

SURGICAL PLICATION OF THE PROTEIN MEMBRANE
(PEYRONIE'S PENILE PLICATION SURGERY)

Plication of the protein membrane of the penis is always a surgery which shortens the penis. There is a special shell made of connective tissue inside of the penis, which is called the protein membrane.

To eliminate the curvature, the surgeon shortens "lifts" the protein-membrane on one side. With this purpose, the fragment of the membrane can be cut out and the edges of the defect pulled (Nesbit operation), or create a fold on it.

Surgery is performed on an outpatient basis, under conductive anesthesia. Usually, a couple of hours later, the patient can go home and do the usual things.

A disadvantage of this method: practically always, the penis is shortened by 1-2 or even more centimeters. This is why the shortening method is used only if the curvature is not more than 60%.
Advantage of this method: with plication of the protein membrane of the penis, comparing to the corporoplasty, the possibility of worsening of the erectile function is significantly less.

PENILE CORNOPLASTY

Cornoplasty is the lengthening methodic of surgical corrections for Peyronie's disease.
During cornoplasty, the incision on the protein membrane is made, on the side where the penis was shortened.
The edges of the defect are stretched and the transplant is installed.
The result: The straightening of the penis occurs without the loss of length. Lengthening surgeries are used if the curvature is more than by 60% if the penis was initially short, or when the male is strongly against its shortening.
The disadvantage of this method: Comparing to the shortening methodic, after the surgery, problems with erection occur more often. The surgical invasion takes more time; it's more complex and expensive.

The use of implants, penile prosthesis. This is the most complex method of surgical treatment that is used when the Peyronie's disease is combined with erectile dysfunctions, which were not solved by other methods.
Implants of cavernous bodies are inserted into the penis. They are the ones when filled with blood, cause the erection. Semi-rigid implants force the penis to constantly stay in a moderate erected state. They are more inexpensive and simple in use but less comfortable for daily usage. Fillable (hydraulic) implants are equipped with a special pump that is sawn under the skin that can be artificially brought to a state of erection.

PENIS STRAIGHTENING WITHOUT SURGERY

With Peyronie's disease, a doctor may prescribe some medications such as Vitamin E, pentoxifylline, and L-carnitine. Data on the effectiveness of drug treatment is uncertain. Some studies showed good results, in others, medications practically didn't give any results whatsoever. There is an opinion that these medications help in stabilizing the situation not allowing worsening of the penis curvature but aren't able to fix it. They are used only in the acute stage.

Sometimes, medications such as Interferon and Verapamil injected directly into the scar plaques. It is believed that they help soften the scar and decrease deformation but some say that the needle causes trauma to the penis during the injection leading to the worsening of the state.

Sometimes a method, analogical to the one that crushes stones in the kidneys and gallbladder, is used. The method is called shock wave therapy. A special generator affects the scar by a series of impulses and softens it. It helps to some men but the shock wave therapy also may cause trauma to the penis and lead to a negative effect.
Penis injections and shock wave therapy can be used with a chronical state of Peyronie's, but these methods are not very effective. Usually, they are used if the curvature is moderate, not more than 45 degrees.

TREATING PEYRONIE'S WITH HERBS

In some cases, quite effective way of treating Peyronie's disease is using the non-traditional medicine. It allows the patient to decrease the symptoms of the disease without having to go through surgery. Such treatment is gentler because it has a weak effect on the male reproductive function. Unfortunately, such treatment works slowly on the patient, so much patience is required.

HERB DECOCTION HERBAL BATH
HERB BATHS WITH BATHMATE
Chestnut decoction
Grind 20 grams of chestnut kernels. Throw it in the boiling water and boil it for 15 minutes on low heat.
Remove from the stove. Let it cool and strain. Drink 1 glass of decoction per 24 hour period.
Medicated baths
It's recommended to use the sage herb.
To prepare the bath, use 3 portions of medicinal herbal collection and pour the boiling water over it.
Let it sit, strain, and pour in the tub.
If you don't have a bathtub or you don't like taking it every day, use the Bathmate hydropump and a Shower Strap.
Pour the warm, dilute decoction into the hydropump and use it on the low resolution sufficient to secure the hydropump. The LNP-therapy, warming of the penis, increase in the blood circulation and medicinal decoction will increase each other's effect.

If the male, suffering from Peyronie's disease, turns to the methods of untraditional medicine, he has a chance to avoid the surgery. Treatment of Peyronie's with herbs provides resorption of formations in the area of the penis and restoration of its normal functioning.
Do not underestimate as well as overestimate the possibility of treatment by herbal collections. Depending on the reason of the disease and its course, such methods may heal the Peyronie's completely or leave without any results whatsoever. It is all individual, but it's worth a try since it is the safest method of treatment. But, do not neglect the medicine if you see that disease progresses! Consultation with a specialist, a doctor, is the correct way of solving problems, especially if you cannot solve them yourself.Watching the illness progress and not doing anything is the worst scenario possible.

STRAIGHTENING WITH EXERCISES

If the penis curvature is moderate, it's possible to fix it with a massage. The success depends on the consistency and correct way of doing the massage.

1.  Make your penis erected and with one hand hold it at its base to slow down the blood flow. You can also use the erection ring (cock ring) to slow down the blood flow.
2. Tilt the penis in the opposite direction to the curvature.
3. After 15-20 tilts, hold the resulting angle in the opposite phase of the bend for 2-3 minutes. Stretching the tunic, the protein membrane of the penis, control the painful sensations and do not allow any dull and sharp pain.
4.  Repeat this exercise 3-5 times.
5. Every time, always, remove the erection ring or relax your hand to renew the blood flow and prevent the oxygen starvation of the penis tissues.
6.  Do this exercise carefully, every day, during 6-12 months until the desired result is achieved. Always prepare the penis before exercise by milking and stretching it for 3-5 minutes to prevent any injuries. Be careful and do not rush the result.

In all types of sport and training, to achieve the results, you must be patient, careful, and consistent in exercising. The straightening of the penis with exercises is no exception to the rule.

PENIS STRAIGHTENING WITH EXTENDER

 Harmful and potentially dangerous idea found on websites and forums:
Straightening the curvature of the penis by extenders and stretchers is possible due to a long, continuous, and longitudinal stretching of the penis. The short side is subject to prolonged stretching which is why the curvature of the penis doesn't go away.

So, how are things really? Is it possible to straighten the penis using the extender?

You can get a temporary, minor effect of penis straightening but just as long as you wear the extender. It's explained by the minor stretching of the shorter side of the penis. The more distorted the penis, the more pronounced the effect of penis straightening will be.
As soon as you stop wearing the extender, all the achieved results will disappear in a matter of 1-3 days. Besides, the curvature may increase due to the fact that you caused additional damage to the tissue of the penis by the extender.There are dozens of examples, reviews from doctors and users, stating that after wearing the extender, the penis developed crookedness.
The longer the extender or the stretcher is worn, the more chances of developing Peyronie's disease and other illnesses associated with degradation of penis tissue due to the constant, malicious hypoxia (oxygen deficiency) of cavernous bodies and other tissues of the penis.
Pay close attention: wearing the extender or the stretcher doesn't help Peyronie's but on a contrary worsens the illness, stimulating proliferation of fibroblasts in the local inflamed area, provoking fibrosis of the penis, seals and excessive growth of connective tissue in the inflamed area. The worst-case scenario may be the degradation of cavernous tissue and as a consequence, prosthesis of the penis.

TREATMENT OF PEYRONIE BY A VACUUM PUMP

Any pathological processes leading to the insufficient nutrition of penis tissues oxygenation of the cavernous tissue, disorders of carbohydrate (hyperglycemia) and fat (hypercholesterolemia) metabolism cause generic damages to the endothelium of the penile vessels and smooth muscles of the cavernous bodies, which is called the - endothelial dysfunction.
With endothelial dysfunction, synthesis of endothelial relaxation factors - nitric oxide and prostaglandins, is drastically suppressed. In these conditions, increased development of endothelin -1 causes harsh contraction of trabecular cavernous tissue's smooth muscles, preventing vasodilatation, which worsens the oxygen deficiency of the cavernous bodies.
Besides, the activation of the factor of transformation B1, the formation of which inhibited by prostaglandins, induces the synthesis of collagen and its accumulation in the cavernous tissue leads to its fibrous transformation. The disorder of elasticity of cavernous tissue and its trabeculae, with the development of fibrosis, leads to the violation of the veno-occlusive mechanism of erection and erectile dysfunction.
This way, sclerotic changes of cavernous tissue are the key and universal mechanism of the appearance of Peyronie's disease and in most cases, erectile dysfunction.
The lowered partial oxygen pressure in the cavernous bodies, inhibition of nitric oxide and prostaglandin synthesis, starts the process of sclerosis of cavernous bodies in the conditions of appeared hypoxia.

Due to the vacuum pump, the arterial blood, saturated with oxygen, rushes into the cavernous tissue, which leads to a drastic rise of the partial pressure of the oxygen, stimulation of development of nitric oxide and prostaglandins, as well as preventing the synthesis of molecules, which induce the cavernous fibrosis.
Increase of blood supply to the penis. The arterial blood, in conditions of lowered barometrical pressure, more intensively saturates the surrounding tissues with oxygen contained in the blood, as well as nutritious elements, hormones, and enzymes. It affects beneficially on the trophy of the tissues of the penis and contributes to the liquidation of degenerative changes in the vascular apparatus of the cavernous bodies and the protein membrane of the penis.
Elimination of hypoxia of cavernous bodies normalizes the mechanisms of synthesis of nitric oxide and prostaglandins and prevents the mechanisms of development of cavernous bodies' fibrosis.

PENIS STRAIGHTENING WITH BATHMATE

All the advantages of using the LNP-therapy for treating the Peyronie, which are listed above, apply to the Bathmate hydropump. The Bathmate hydropumps have a list of advantages comparing to the vacuum pumps for the penis and are considered a new round for the evolution of vacuum-erectors for the LNP-therapy.

MAIN ADVANTAGES OF BATHMATE VS THE AIR-VACUUM PUMP:

1. THE OPPORTUNITY OF INDIVIDUAL SELECTION OF HYDROPUMP DEPENDING ON THE SIZE OF THE PENIS
The opportunity of individual selection of hydropump plays a very important role to increase the effectiveness of forceful straightening of the penis. The walls of the hydropump are directing during the straightening of the penis. If the hydropump is purchased exclusively for the penis straightening, it is very recommended to pay close attention to the sizing chart and do not purchase a Bathmate you will "grow into".

2. EXCEPTIONALLY PRECISE CONTROL AND DILUTION SPEED
In the air-pump, for the penis to begin to give in to resolution, it's necessary to pump out the air for a long time, stretching it in volume. Only after 5-10 pump compressions, the necessary resolution can be achieved, to make the penis straighten under the action of forced erection. In a hydropump, it is enough to squeeze out the necessary volume of water by pressing once on a hydropump and the penis will respond by the increase of the penis' size due to the compensation of rushing blood to the cavernous body. Since the water is not subjected to stretching, the penis's response to pumping the water out of the hydropump will be instantaneous.

3. WARMING THE PENIS WITH WATER
In air pumps, there is no preparation of the penis for stretching. The hydropump, on the contrary, contributes to the relaxation and preparation of the penis towards stretching, increasing the blood circulation in the skin of the penis. Due to this effect, there are almost no side effects which are usually caused by air pumps, such as skin darkening, increased swelling, bruising, and dots. Warming of the penis contributes to the larger progress in the increase and straightening of the penis.

4. THE POSSIBILITY OF TILTING THE HYDROPUMP
Due to the construction of the sylphon-pump, it is possible to tilt the hydropump in all directions. If the curvature of the penis is localized closer to its base, then it is possible to tilt the pump in the opposite phase of curvature. This method is more suited for the congenital curvature of the penis. If you have the acquired curvature associated with the formation of the calcified plaque, we don't recommend that you overstretch this area. First of all, you need to stop the process of plaque growth and increase its resorption, increasing and restoring the blood circulation in this area with the LNP-therapy and Bathmate hydropump.

5. THE ABSENCE OF EXCESSIVE BENDING OF THE PENIS AT THE BEGINNING OF TRAINING
The water is the substitute for the lubricant. During the immersion of the penis into the air-pump, the penis might get stuck sticking to the walls of the air-pump. The penis goes into the hydropump much easier, practically not getting stuck to the walls of the flask, which prevents any additional over bending of the penis and worsening of the curvature with which the user is fighting against.

6. THE POSSIBILITY OF USING THE LNP-THERAPY TOGETHER WITH NECESSARY SOLUTIONS AND DECOCTIONS
Since hydropump uses the liquid as a working environment, there is a possibility of using the LNP-therapy together with prescribed solutions or herbal decoctions. Simply pour the required liquid into the hydropump, dip the penis into it, quickly turn it over, pressing the hydropump towards the body, press on the pump to displace the excess fluid and fixate the hydropump on the body.
