Peyronie's disease –
is a pathology that is typical only for the males.
The disease is characterized by the appearance of painful erection due to the fibrous changes in the protein membrane.

In some cases, this illness can have a negative impact on conception but it's present only in those cases where the penis is curved so much that it cannot enter the female sexual organ.
This is why it's necessary to see a specialist after noticing the first symptoms (the presence of a small seal that is easy to detect by touch, painful sexual intercourse, and deformation during each erection). The treatment is surgical.
THE REASON FOR APPEARANCE OF PEYRONIEThe reason for the appearance of Peyronie is unknown. Most likely, it is due to the repeated microtrauma of the protein membrane of the cavernous bodies. The disease starts with an acute inflammatory process in the result of which dramatically increases the proliferation of fibroblasts of the membrane, some of which differentiate into myofibroblasts, increases the deposition of collagen and fibrin. Ongoing inflammation leads to remodeling the connecting tissue into the fibrous plaque which may lead to the curvature of the cavernous bodies.Risk factors for Peyronie's disease are diabetes, arterial hypertension, lipid metabolism disorders, coronary heart disease, erectile dysfunction, smoking, and alcohol abuse. THE PROCESS OF PEYRONIE'S DISEASEThere are two stages of this disease: the stage of acute inflammation and the fibrotic stage.
The stage of acute inflammation can reveal itself with the pain in the penis at rest, painful erection, as well as the appearance of "soft" plaques and curvature of the penis. In the fibrotic stage, a dense palpable plaque is formed, which can subsequently calcify, which indicates the stabilization of the disease. With time, the worsening of the condition was detected in 30-50% of patients and stabilizing in 47-67%. Spontaneous improvement in the patient's state happens only in 3-13% of cases. Improvement is possible only in the early stages of illness. After formation, especially if the fibrotic plaque has been calcified, spontaneous improvement happens very rarely. A first diagnosis is made based on the results of the external examination and palpation of the penis.
Diagnoses confirmation can be made after the x-ray or ultrasound of the penis which determines the size and structure of the fibrous plaques.

It should be noted that in some cases the penis is so curved that a simple sexual act becomes practically impossible and causes serious discomfort to a partner.
In this case, newly formed plaques with the consistency of cartilage can interfere. They may get up to 3 cm in size. Usually, large plaques are in the protein-membrane closer to the body of the coronal groove of the penis, close to the pubic area. The illness has two stages:
When should you see a doctor? See the urologist-andrologist if you notice that your penis is curved and it gets in the way of having a sexual act or causes pain and erection dysfunction. You must take a photo of your penis when it's in the stage of erection, from the top, to the side, and in front and bring the photos to a doctor. A doctor will take a look at the genitals and if needed, will inject the substance into the penis that will cause the erection to see it in the state of erection and determine the level of curvature. For scar-tissue detection, an ultrasound or MRI is prescribed. |
Plication of the protein membrane of the penis is always a surgery which shortens the penis. There is a special shell made of connective tissue inside of the penis, which is called the protein membrane.
To eliminate the curvature, the surgeon shortens "lifts" the protein-membrane on one side. With this purpose, the fragment of the membrane can be cut out and the edges of the defect pulled (Nesbit operation), or create a fold on it.
Surgery is performed on an outpatient basis, under conductive anesthesia. Usually, a couple of hours later, the patient can go home and do the usual things.
A disadvantage of this method: practically always, the penis is shortened by 1-2 or even more centimeters. This is why the shortening method is used only if the curvature is not more than 60%.
Advantage of this method: with plication of the protein membrane of the penis, comparing to the corporoplasty, the possibility of worsening of the erectile function is significantly less.
PENILE CORNOPLASTY Cornoplasty is the lengthening methodic of surgical corrections for Peyronie's disease.
During cornoplasty, the incision on the protein membrane is made, on the side where the penis was shortened.
The edges of the defect are stretched and the transplant is installed.
The result: The straightening of the penis occurs without the loss of length. Lengthening surgeries are used if the curvature is more than by 60% if the penis was initially short, or when the male is strongly against its shortening. The disadvantage of this method: Comparing to the shortening methodic, after the surgery, problems with erection occur more often. The surgical invasion takes more time; it's more complex and expensive. The use of implants, penile prosthesis. This is the most complex method of surgical treatment that is used when the Peyronie's disease is combined with erectile dysfunctions, which were not solved by other methods.
Implants of cavernous bodies are inserted into the penis. They are the ones when filled with blood, cause the erection.
Semi-rigid implants force the penis to constantly stay in a moderate erected state. They are more inexpensive and simple in use but less comfortable for daily usage.
Fillable (hydraulic) implants are equipped with a special pump that is sawn under the skin that can be artificially brought to a state of erection.

Sometimes, medications such as Interferon and Verapamil injected directly into the scar plaques.
It is believed that they help soften the scar and decrease deformation but some say that the needle causes trauma to the penis during the injection leading to the worsening of the state.

Sometimes a method, analogical to the one that crushes stones in the kidneys and gallbladder, is used. The method is called shock wave therapy. A special generator affects the scar by a series of impulses and softens it. It helps to some men but the shock wave therapy also may cause trauma to the penis and lead to a negative effect. |

Chestnut decoction
Grind 20 grams of chestnut kernels. Throw it in the boiling water and boil it for 15 minutes on low heat.

If the penis curvature is moderate, it's possible to fix it with a massage. The success depends on the consistency and correct way of doing the massage.
1. Make your penis erected and with one hand hold it at its base to slow down the blood flow. You can also use the erection ring (cock ring) to slow down the blood flow.
2. Tilt the penis in the opposite direction to the curvature.
3. After 15-20 tilts, hold the resulting angle in the opposite phase of the bend for 2-3 minutes. Stretching the tunic, the protein membrane of the penis, control the painful sensations and do not allow any dull and sharp pain.
4. Repeat this exercise 3-5 times.
